2) In the Personnel unit of the agency where it provides services, filling in the official form below.

3) By postal mail, filling in the aforementioned official form.

4) In any of the records provided for in article 16 of Law 39/2015, of 1 of October, of the Common Administrative Procedure of Public Administrations.

If the IRPF model 145 is not presented, the type of retention will be calculated without taking into account their personal and family circumstances (art. 88.2 of the Regulation on Personal Income Tax).

The staff body shall inform MUFACE of the fourth month’s leave together with the certificate of the basic and additional remuneration accrued at the beginning of the third month of leave and of the additional remuneration not paid from the 91st day.
